In developmentSettings → Get reimbursed will produce a one-page PDF laid out the way an expense form asks. The PDF goes with Apple's emailed receipt, which remains the proof of payment.
Send both to your coordinator
Hand in both documents and say what the software is for.
What is on the receipt
In developmentSeller. Apple, the merchant of record on an App Store purchase, with Capnolog named as the product. On a form with one vendor box, put Apple there and Capnolog in the description.
Product. The app and the exact tier bought, described as educational software.
Date. The date of the purchase.
Amount. What was charged, in your Apple account's currency.
Transaction reference. The identifier that matches the PDF to Apple's receipt.
Purchaser. You. A receipt covers one purchase by one person, so a co-resident buys and claims their own.
Ask first. It is a short email.
